I would recommend having the different sections actually mean something, rather than just adding it up to a final score. One section could be used to determine immediate overdose levels, one for long-term use, and the third for special conditions.
Immediate overdose would be taking a single massive amount of the drug at once, so the question would be if it has the ability to act like a poison at that moment, with potentially rules for deciding how severe it is.
Long-term usage could require a Constitution role after a given period, dictated by the drug itself, and failure checks off a box in this section (Critical Failure could be varied, from immediate reaction to just checking off two boxes). Once this block fills up, the drug is treated as a poison of some kind. It should also be possible to "heal" this section, with some exceptions, so a character can recover boxes over time.
Special conditions could include something like building up to odd reactions or even detectability.
