Physical attempts to unlock them result in curses. To open them requires a ritual. This may be simple or incredibly complex in terms of time and resources, the latter used for locks that are opened only once in a great while.
Trollform is a spell that allows the recipient to heal and regenerate quickly. The downside is they act like a troll for the duration.
Greater fiends can turn lesser ones into gear. For example, the sword a pit fiend was wielding was broken, so it then turns a bearded devil into a spiked mace as a replacement. This isn't a quick transformation, it should take at least 10 minutes, and so is rarely used on the battlefield*.
* An ironic statement considering real battles could last for days.
Some planar critters are mutagens but are themselves in turn mutated by the presence of those they transform.
Some special buildings are hungry small gods. If they are not fed, they will transform and leave or rampage. Consider how bad this would be if the build is the castle that protects people from ravenous hellhounds in the surrounding forest.
Large rooms can be multiple locations- each wall, ceiling, floor, alcove, etc. is treated as a different place to explore and interact with. Sometimes this means all the surfaces are treated as floors, at least for critters that can walk on them.
