A quick example using a cybernetic limb comprising a lower human arm (forearm below the elbow, wrist, and hand).
A PC gets a new cybernetic limb. It looks like a cybernetic limb, so is obviously not flesh and blood, but has an overall looks like a mechanic device intended to mimic a human arm, and has an outer polymer casing.
Over time, the character bangs their arm around, so it gets beat up. They also spend a lot of time tightly gripping objects, whether during climbing, combat, or other circumstances (doesn't really matter).
Over that time, the limb begins to evolve in response to this use. The pinky finger begins to rotate to form a second thumb for better gripping. Small biometallic claws form on the finger pads to allow for better climbing. The outer polymer casing (more like the exoskeleton of an insect originally, with the nanites forming it out of a polymer) starts to look like alligator or crocodile skin as they begin to form biometallic osteoderms to add additional protection. And, finally, the limb has expanded, first using metallic nanite tendrils and then even replacing organic components of the rest of the arm, so it now is almost up to the shoulder joint. At that point, no doctor with any sort of medical scanner can tell the difference between the cybernetics and the human.
Other options could be that the nanites begin to extend themselves outward into other cybernetic components already present, causing them to be altered to other standards.
It should remain symbiotic, though, and I would generally say that the character should be the overriding intelligence, but I can definitely see a Venom potential in there. A grey goo cybernetic implant could be intelligent enough to act like an intelligent magical item, and could either form a corrupting or positive relationship with the character.
