People have a secret super power. When a group finds a dead world, a rock that has yet to develop life, they can dream a new biosphere and a few locations into existence. The danger of this is obvious- nightmares and egos warping the vision- and so it is rarely done even by those who know of this power.
Space whales, Great Dreamers from Spelljammer, can also start life on dead rocks by diving into the oceans. If there are no seas, then the whales can create them and then swim, starting life on those worlds.
All living worlds had an egg as a moon. When it hatched, and the world was populated. The fragments are valuable to healers and vivimancers as they act as material components that are never consumed.
Some islands are centered on volcanoes that emit gas that kills and reincarnates everything to the shores and possibly even into the nearby seas.
Hybrids of dragons and elementals are novel species. The parents have no control over what the offspring are and usually they are birthed as groups of 20+, enough to found new species populations for the world.
When sand is used to make glass, the blood of desert dragons (blue/brown/yellow) can be added to turn the glass into one way mirrors. The odd thing is they never reflect reptiles.
