The troll kills another creature and then replaces one of its victim's limbs with one of its own arms. The arm's flesh continues to regenerate and its flesh overwhelms and replaces the victim's. Within a day or week a new troll arises. This can be done with almost any living thing that is troll-sized or smaller. They only do this when their population drops, allowing them to feed much more often than reproduce.
Spirit trolls, the abominations that were created by crossing trolls with invisible stalkers, can also graft some of their flesh but they do this with permanent magic items. The items are turned into new spirit trolls that can use the item's power.
The mind abhors a vacuum and that is why the Astral plane is filled with hallucinations rather than mostly empty voids.
The early gods took different elements from several animals and used them to make humans. If the animal kills a human, it gets that trait back. That is why wolves hunt humans- to get their ability to reason back. (And whatever other traits you want to use.) This is not inheritable and so humans will always be looking out for those specific animals.
There are mice-sized people that use geared machines to use the strength of humans. They are very good at designing these devices and many other of the tiny folk commission their works.
Monsters usually come from underground, the deep seas, space, and other planes. There is another possibility for some settings- the mind. When a psychic dies there is a small chance they leave behind a thoughtform. All of these are monsters but as they draw on the progenitor's personality traits, not all of them are evil. Each may be a unique creature or the start of a new species of creatures.
The oceans are prisons for epic monsters. Their corpses little the deepest sea floor, chained to the core of the world. If the water is removed, the revive, shatter the chains and walk the world once again. The same can be applied to forests and daikaiju. Clear cut and watch the ground erupt into world ending horror.
